Here is the "smoking gun" of events leading to the rocket's demise.  The rocket neared mach at 1000 feet which critically changed aerodynamics around the front bay closure.

Notice the osculation in the smoke trail. It became critically unstable when bow shock and mach waves formed around the nose and bay leading/trailing edges. If you watched old videos of test planes trying to achieve mach, you will notice the same osculations that would lead to failure. The thickness of the front fins (enclosure for phone) is what could not be computed properly by the simulators and they warned that their math could be wrong and this was a huge concern of mine.

The field term for this type of failure is "shred" as the rocket disintegrated. In this case, due to the instability. Notice the nose cone or lack of most of it. Only the broken tip remains. A nomex (kevlar) square is the odd cloth on the cord between the nose and camera and is also shredded. The strings near the nomex square are what is left of the drogue parachute. The grey and orange lines are the remains of the wiring to the deployment charges. And the smoke trail shows the osculation. The wind deformed the trail somewhat so it is not more of a vector.
